PURPOSE: To lead an outreach team to develop Integrated care pathways
for Travellers to access addiction and rehabilitation services in
North Dublin area.

Coolmine is a leading drug and alcohol treatment service providing
community, day and residential services to men and women with
problematic substance use and their families in Ireland established
since *. Coolmine Therapeutic Community

addiction and lead a fulfilled and productive life.

KEY RESPONSIBILITIES: (INCLUDE, BUT NOT LIMITED TO)

* To develop and lead out on an assertive strategy to engage with
Travellers and their families in the North Dublin Area.
* Liaise closely with Traveller Development and Advocacy Groups and
oversight committee to increase a more strategic and cohesive approach
to delivering addiction services to travellers.
* Proving leadership and line management to Outreach Team staff with
regular and consistent communication, including team meetings and
supporting the ethos, philosophy and methodology of the Coolmine.
* Provide case management and 1:1 supports to clients and their
Families consulting with them and enabling them towards positive
progression
* Oversees assessments with client case load, in order to direct
comprehensive Care Plans that address clients’ needs with a system
of regular review.
* Develop a strong referral pathway to services that provide
additional support services including but not limited to: mental
health services, advocacy, counselling, housing and human rights
services.
* To promote strong collaboration and interagency working between
all services working with travellers and their families affected by
addiction
* Responsible for building the capacity of peer support workers in
line with the Project aims through dedicated training plans and
mentorship
* Develop and maintain clear reporting structures including activity
reports, statistical and NDTRS returns and gaps and blocks
* Review and evaluate the effectiveness of the services in
collaboration with the Community and Day Services Manager through
mediums such as internal statistics, emerging trends, client and staff
feedback and audits responding to presenting client need and making
recommendations for future service development.
* Report to the Community and Day Services Manager for individual
support, supervision, performance appraisal, and client case review;
and working within the parameters of the Coolmine HR policies and
procedures.
* Ensure the continuity and coordination of the person-centred
health and social services through monitoring and supporting staffs
case management.
* Work with Community & Day Services Manager to ensure their
delivery in all areas of Human Resources with Performance Management,
Communications and staff training.
* To comply with all Health and Safety regulations and standard
operating procedures relating to fire procedures, maintenance,
critical incidents, safety for staff, etc. to ensure the effective
safety and well-being of all clients and staff.
* To work within the policies of Coolmine Therapeutic Community in
order to ensure that a consistent delivery of service, quality
standards and best practice are adhered to at all times.

ADMINISTRATION SKILLS

* Proficient in Case Management systems. E.g. Ecass
* Effectively implement work plans.
* Maintaining clear reporting structures and Keeping records
pertinent to the post and observe professional standards ensuring
confidentiality of records and security of same.
* Administer the day-to-day aspects of the service and facilities.
* Provide reports and statistics as requested.
* Participate in ongoing evaluation and in initiatives to improve
the quality of all aspects of the service.

PERSON SPECIFICATION

EDUCATION

* Third level education in relevant Social Care field (Minimum
Recognised Level 7 Diploma )

EXPERIENCE

* Minimum Three years’ experience working within the drug and
alcohol field.
* Experience of working with Travellers would be a distinct
advantage.
* Line management and Supervision experience
* Familiar with various models of rehabilitation, counselling, and
recovery approaches.
* Two years’ experience of the implementation and evaluation of
care planning/Case management
* Experience of coaching and mentorship is desirable
* Experience of working as part of a multidisciplinary team and
interagency working within addiction services.
